  Business Patron Drive Form
    This form is used for a business to purchase a business patron for the SJCC Yearbook Patron Drive.
  Email Request Form
    Please complete this form and send it to SJCC so we may have your email on file for communications.
  Emergency Medical Authorization
    Please complete this form for our records in the event of an emergency.
  Individual Patron Drive Form
    This form is used for an individual to purchase a patron for the SJCC Yearbook Patron Drive.
  Internet Picture Release Form
    Please complete this form and send it to SJCC, giving us permission to use your son or daughter's picture on our website.  
  Military Information Release Waiver  
    This form states that you do not wish your son or daughter's personal information to be released to any member of the military.  
  OHSAA Physical Form  
    Print out this form and complete it. Then turn in to the SJCC office. This is required for all students playing sports at SJCC.  
  Open Lunch Form  
    This form gives your son or daughter permission to leave campus for open lunch.  
  Service Hour Form  
  Download this form if you need copies to fill out to fulfill the SJCC Service Hour Requirement.
